Joined Up Care Derbyshire
Joined Up Care Derbyshire, also known as Derbyshire’s Sustainability and Transformation Partnership (STP) brings together health and social care organisations across Derbyshire. Working together more closely than ever before, the ambition is to provide the best care and services for people and make them as efficient and effective as possible.
The plans set out a vision for a more joined-up approach to health and social care, the steps that should be taken to get there and how everyone involved needs to work together to improve what we deliver.
Health and social care organisations in Derbyshire have been working closely together for some time, to improve care and services for people and make them as efficient and effective as possible. Our plans have been reviewed during summer 2019 - you can see the progress made in our plans section.
What is Joined Up Care Derbyshire?
NHS and social care teams do a superb job caring for record numbers of people.
Quite rightly, people to want to protect our health and care services when they are under pressure but change and improvements will also need to be made over time.
Since 1948, the NHS has constantly adapted itself and it must continue to do so as the world and our health needs also change. We are now able to treat people with new drugs and treatments that were not available in the past. As life expectancy increases, so do the ailments of old age and there are now more people with long-term conditions.
There are also big opportunities to improve care by making practical changes to how the NHS works. Improvements that matter, like making it easier to see a GP, speeding up cancer diagnosis and offering help faster to people with mental ill health.
This is why the NHS, local councils and the voluntary and community sector have come together in 44 areas covering all of England to develop proposals to improve health and care. They have formed new partnerships – known as sustainability and transformation partnerships – to plan jointly for the next few years.
Our local sustainability and transformation partnership is known as Joined Up Care Derbyshire. It brings together twelve partner organisations and sets out ambitions and priorities for the future of the county’s health and care. All the organisations that provide health and care aim to work and plan much better together, focusing on new ways of working to:
- Help keep people healthy
- Give people the best quality care
- Run services well and make the most of available budgets
